Maternal Infant Health Program (MIHP)

Position Overview

Join the Maternal Infant Health Program (MIHP) as a Home Visitor Social Worker and make a meaningful impact on the lives of pregnant women, new mothers, and infants. As an essential member of the MIHP team, you will provide in-home assessments, case management, education, and support services to families, helping them navigate challenges and meet their health and developmental goals. This role requires a compassionate, dedicated social worker with excellent communication skills and a strong commitment to empowering families.

Key Responsibilities

Required Training: All service providers, including independent and unpaid providers, must complete all required MIHP trainings at their own expense and on their own time, without compensation. Proof of completion must be provided before beginning services and upon request.

1. Home Visits and Assessments

Conduct in-home or community visits with pregnant women, postpartum mothers, and infants.

Perform comprehensive psychosocial assessments to identify strengths, challenges, and support needs.

Evaluate home environments to ensure the safety and well-being of the mother and child.

2. Case Management and Support

Develop and implement individualized care plans to meet each family’s specific needs and goals.

Provide counseling and support on mental health, family dynamics, and other psychosocial concerns.

Assist clients in accessing community resources such as housing, food assistance, transportation, and baby items, providing referrals when needed.

3. Education and Advocacy

Educate families on topics such as prenatal care, infant health, breastfeeding, safe sleep practices, and child development milestones.

Advocate for clients within healthcare and social service systems, ensuring they receive necessary services and support.

4. Collaboration and Coordination

Collaborate with a multidisciplinary team, including nurses, dietitians, infant mental health specialists, and other MIHP team members to deliver comprehensive care.

Coordinate and communicate with healthcare providers, social service agencies, and community partners to ensure integrated care for clients.

5. Documentation and Reporting

Maintain accurate, timely, and confidential client records according to program guidelines and regulatory requirements.

Document all home visits, assessments, interventions, and outcomes.

Prepare reports for program evaluation, compliance monitoring, and outcome tracking.

Qualifications

Education: Bachelor’s or Master’s degree in Social Work (BSW, MSW) from an accredited institution.

Licensure: Current licensure as a Social Worker in Michigan (LBSW, LMSW).

Experience:

1+ years of experience in maternal and child health, social work, or home visitation programs preferred.

Knowledge of community resources and social determinants of health is highly valued.

Skills:

Strong interpersonal, communication, and organizational skills.

Ability to build trusting relationships with diverse families and communities.

Proficient in documentation and meeting deadlines.

Basic computer literacy (e.g., Microsoft Office, electronic health records).

OTHER REQUIREMENTS

Valid driver’s license and reliable transportation for home visits.

Flexibility to work non-traditional hours as needed.

Work Environment

The position requires traveling to clients’ homes and community settings.

Occasional lifting of materials or supplies may be required.
